Why we ask for ID

For client safety, accurate recordkeeping, and fraud prevention, all new clients are required to present a valid government-issued photo ID at their first appointment.

Clients under 18

Clients under 18 must have a parent or legal guardian present with identification. This helps us verify consent and ensure the right treatment decisions are made for younger clients.

What forms of ID are accepted

• Driver's license
• State-issued ID card
• Passport
• Other government-issued photo identification
